The Military Transition Support Project (MTSP) began as a planning project convened by the San Diego Grantmakers Military Family Support Working Group and a broad coalition of community leaders and organizations. This coalition gathered the input of hundreds of community members and incorporated national best practices to develop a comprehensive plan to connect San Diego’s sectors – military, social services, public officials, employers, education and philanthropy. The plan focused on supporting transitioning military families 9 months prior to leaving the military through 12 months post-service.
In San Diego, we are convinced that transitioning service members should be better off for having served their country. We stand together to pause, salute, and support military members and their families.
The implementation of the Military Transition Support Project is called zero8hundred. This name illustrates the promise that comes with a new day, and captures San Diego’s resolve that it is time for a bold new approach that will transform the transition experience for service members and their families. Time to bridge the gap, and leverage the broad array of resources that are available to ease transitions for those who will be joining our community.

The details - how will zero8hundred achieve this?
zero8hundred uses a Veteran’s Wellness Framework with four key pillars: Education and Jobs, Basic Needs, Physical and Mental Health, and Personal/Social Connections. The challenges of transition are managed through establishment of a focused web portal and proactive interaction with experienced peer navigators, addressing vital employment, education, health and wellness issues.
zero8hundred’s comprehensive easy-to-use Internet portal provides information on services and resources specific to the San Diego region. This bridges the gap between transitioning members and resources, and it is available to anyone seeking transition resources in San Diego.
zero8hundred provides access to experienced peer navigators who work closely with transitioning service members and families. Through a unique partnership with the armed forces, zero8hundred is offered during transition classes at Navy, Marine Corps, and Coast Guard bases in the San Diego area. The program is also presented to military members and spouses at select community venues. Transitioning members at these classes have the option to sign up for periodic interaction with peer navigators. Once they opt-in, they become zero8hundred “VIPs” (for veterans or spouses in process), and navigators proactively call them from 9 months before until 12 months after separation to discuss transition challenges. zero8hundred provides ongoing support by linking VIPs and their families to a broad range of resources and opportunities to help them successfully transition into civilian life.
